    Hehe. My roses~

    Red: Red is the symbol of love. To give red roses says "I love you still" 
    White: Shows purity and innocence. They also represent humility, youthfulness and charm. 
    Pink: To give pink roses shows grace, joy, and gratitude. Pale pink also has the meaning of fun attached to it. 
    Orange: Desire and enthusiam. These would be a great choice to give to someone if you want the relationship to develop further. Lilac and Lavender roses also show these feelings. 
    Yellow:Joy and friendship - yellow roses don't specifically show love. They can also represent starting over,or a new beginning. Yellow tipped with red shows friendship falling in love.

    12:The standard for "be mine". Often given on Valentines day, when for a married couple,actually 7 or 10 might be a better choice.

    The surprise came at around 11:12am. Its a couple bear with 12 pink roses!!! My favourite pink! :) Haiya Dear not around to see me smile. I'm smiling since just now for the past 5 minutes le. Lolx... The second bouquet of roses from Dear. :))

    Diabetes Mellitus

    This is a common endocrine disorder all over the world. 9% of 140 million prevalence rate in Singapore. It is a heterogenous chronic metablolic disorder characterised by presence of hyperglycemia due to lack of insulin.

    It can be due to decrease of insulin, decrease to response to insulin or increase in counter-regulatory hormones.

    The chronic hyperglycemia is associated with long-term sequela (an abnormal condition resulting from a previous disease) results in damaging the kidneys, eyes, nerves and blood vessels. These damges is slow and progressive.

    It is classified into primary and secondary causes. Primary causes are Type 1 Diabetes (IDDM)- Insulin Depend DM. It is due to destructive of beta cells(autoimmune) or very little or no insulin production. For Type 2 Diabetes (NIDDM)-Non-insulin depend DM, it is due to insuffiecient or insulin resistance (beta cells dysfunction).

    The secondary causes are due to pancreatic disease(pancreatits), liver disease(liver cirrhosis) and excess secretion of insulin antagonist (glucocoticoids, catecholamines and growth hormone).

    Clinical features: Polydipsia(excessive thirst), Polyphagia(increased hunger) and weight loss, even when eating well.

    Pathophysiology: Insulin promotes glucose entry to the cells. In DM, due to lack of insulin=cells can't absorb glucose.

    The acute complications: Hyperglycemia, hypoglycemia and diabetic ketoacidosis.

    Late complications: Ischaemic heart disease, stroke, peripheral limb ischaemia, eye retinopathy, kidney nephropathy, nerve nephropathy, skin lesion (esp feet-due to damage to bld vessels resulting in foot ulcers and gangrene of toes.)

    I woke up at 7plus so decided to bath and head to Dear's house. :) Now I'm waiting for my surprise. I'm studying Bio now. :) About Crushing syndrome. Again, if you want to know more, continue to read ba...

    It is caused by a chronic exposure to excessive circulating glucocorticoids. Glucocorticoids are a type of steriods. It is due to long term therapy on these steriods medications.

    The main features that these patients have are: Moon shape face, fats being redistributed to only face and the body trunk. They will have very thin limbs and have buffalo hump on the back of their neck. The antagonistic effect of cortisol to action to insulin is hyperglycemia. It will cause diabetes in other words. However if the steriods are stopped, this condition will go back to normal. Its important to check the glucose level.

    The other effects include increased protein catabolism like muscle wasting, thinning of the skin, striae(lines or stretch marks on the skin), easy bruising, osteoporosis, purplish striae on the breast and arms. For the mineralcorticoids effects which is hypertension.The enzyme aldosterone is associated with mineral glucocorticoids which act by reabsorbing water ad sodium. The BP will increase due to the increased work load of the heart to pump harder to excrete the excess water.

    Inhibitory effecs on immune cells which will increase susceptibility to infections. Patients have to avoid crowded places. It will affect the brain cells causing psychiatric effects and hirsutism for androgenic effects.

    I went to school at around 2 plus after lunch with Dear at KFC.
    I started to revise at the Lib and went to take my paper at 4pm. I finished the paper at around 430 and tried to complete the rest that I left blanks. By that time it was near to 445. Thus, I left the hall raising up my hands. Lolx~ First time I leave so early cuz if I stay for the whole one hour, I will leave the hall even later at around 510.
    Went to meet Dear and we had dinner at CS. We had pepper rice and then went around to walk awhile.He ask me to go his house earlier tmr cuz got some delivery people will come his house to give me smth. He asked me if I have anything for him anot... Lolx. I dunno wor... Tmr then you see ba.

    Wednesday. Today. Two more days to Pharmaco Paper! :) Lolx... Not really looking forward but I hope I will be even more confident after tomorrow. Today! I must study later! Now I'm blogging. Just now i was waiting for Joc. Now I'm still waiting so took some time to blog on what I remember... If you want to learn about this anticoagulant drugs then continue to read. :)
    Just read about the effects of Warfarin. It's major side effect is Bleeding. It should not be taken when pregnancy. An example of Warfarin is Coumadin. It has a narrow TI(theraputic index). It will have purple toe syndrome which is a net-like skin rash usually on the hands and feet. Alopecia is the loss of hair but its not common. Warfarin's MOA: It inhibits the enzyme Vit K epoxide reductase, which is needed for clotting factors. Therefore, prevents clotting.

    Heparin is another kind of anticoagulants. Its MOA: Converts fibrinogen to fibrin. Increases the action of antithrombin3. Its major side effects is also bleeding and after taking it for 5 days, some will have low platelets count (thromobocytopenia). Its available as Heparin Sodium and Low Molecular weight heparins(LMWH).
    It should not be taken when pregnancy too.
    Patient Education:
    Use soft tooth brush and electric razor to prevent bleeding.
    Inform other health care workers that you're on Warfarin.
    Do not double your dose if your miss the medication.
    Do not stop the drug suddenly.
    Avoid grapefruit juice.
    Not to eat with other medications unless stated by the physician.
    Avoid stenuous activities or exercise to prevent bleeding.
    Do not change your diet suddenly(eg increase or decrease intake of vegetables, it can affect the clotting factors.)
    Overall these drugs do not dissolve the existing clots but the prevent further clots from foming to prevent complications (eg atrial fibrillation).
